MEDICARE HOME HEALTH REFERRAL CHECKLIST


Medicare's definition of "HOMEBOUND": The patient must experience a nonnal inability to leave home. Their physical condition and/or physical limitations are such that it would be a considerable and taxing effort for them to leave home, i.e. requiring the assistance of others and/or supportive devices. Non-medical absences from the home must be infrequent and/or of short duration.

SKILLED NURSING visits are considered skilled/reimbursable by Medicare for:

  • Medication regimen
  • NP/ Trach suctioning
  • Ostomy care

PHYSICAL THERAPY is comsidered skilled / reimbursable if the level of need requires intervention by a trained therapist when the patient has difficulty getting in/out or repositioning in bed, walking (inside or outside), going up or down steps, standing / sitting, or getting on/off toilet or bedside commode OR
Patient has:

  • Contractures or pressure areas
  • Decline in function related to stroke, pain, arthritis, amputation, etc.
  • Equipment needs
  • History of falls
  • Limited ROM, decreased sensation or strength
  • Poor safety awareness
  • Shortness of breath with activity

HOME HEALTH AIDE serivecs can be provided to a patient requiring assistance with activities of daily living if they are also receiving skilled services (nursing or physcial therapy) and has problems with:
