It’s that time of the year again to Save the Date for the Diabetes walk/run. Date: August 15, 2009 Time: 8:00 am Where: The walk/run start line will be at the Monroeville Water Tower. Display booths and a fun time to be had is at Monroeville Square as well as the after walk/run awards ceremony. Per-register at the Foundation office or on-line at www.mchcare.com Late-register 7:00 T-shirt Design contest How to Enter: Completed artwork must be postmarked or delivered by July 1, 2009. Entry must include: * A completed entry form attached to the artwork * Artwork Entry Form for the t-shirt design contest – for the participate t-shirt. Name of Artist: ___________________________________ Phone number: ___________________________________ Address: _________________________________________ If a minor parent’s name: ____________________________ Rules & Regulations: * The design for the Diabetes Walk/Run t-shirt is open to anyone. Only one entry per person will be accepted. * Entries will be judged on the quality of the artwork * The artwork must call attention to the Walking/running or exercising for a better life with Diabetes. All artwork must be the contestant's original, hand-done creation. * Do not sign the artwork. The winner will be allowed to sign artwork after the design has been chosen. * Photographs and computer-generated artwork will not be accepted. * The design will be used for the participate T-Shirt and Poster * All artwork must be 8.5" x 11" without a mat, frame, covers sheet or border. Art techniques may include scratch-board, pointillism, chalk, charcoal, dry brush, watercolor, crosshatch, lead, collage, or crayon. (Please note: if contestants use chalk or lead they should seal it with an adhesive) * If contestant would like artwork returned, it will be available at the Monroe Health Foundation office for pickup from after the walk/run until December 31. If you wish it to be returned by mailed, please include return mailing materials, mailing address, and postage. If those are items are not included in the request for return mail, it will NOT be mailed back to you.
